Athlete Medical & Registration Forms
To fight MMA, Kickboxing & Muay Thai in Manitoba you must be registered with the Manitoba Combative Sports Commission and the following medical requirements must be met: Contestant Checklist Application for Manitoba Combative Sports License MCSC Physical Examination *All medicals and blood work must be within 90 days of contest.
